Join the Team this Summer: You can print an application or call us at (815) 875-8100 and we'll send you one. You can also write us with a request for one or more applications at the address on our contact page.
Option 1: Print an application Print out a blank application that you can fill-out yourself, give to your friends (it's always more fun to work with your friends), hand out to classmates or wallpaper your room. --Don't forget to sign it before you mail it in --and have a parent sign it if you're under 18. Option 2: Print an application for a Crew Leader or Bus Driver Position Use this application form if you're interested in a Crew Leader position (and you are 17 or older) or a Bus Driver position (and you are 21 or older with a CDL that includes a passenger endorsement) Option 3: Contact Us We'll be glad to send as many applications as you need (it's more fun to work with your friends).

Make sure you read the Team Corn Work Agreement. It describes how you will be paid and other conditions of your employment. A copy will also be mailed to you for you to sign. It is also a good idea to read through the Worker Protection Standard. It describes some of the regulations that are designed to keep you safe in the fields. Forms: The I-9 form that ensures the government that you are eligible to work in the U.S. The W-4 form which tells us how much money to withhold from your paycheck to pay federal taxes State Government Required Forms: Illinois W-4 form which tells us how much money to withhold from your paycheck to pay Illinois state taxes lowa W-4 form (44-019b) which tells us how much money to withhold from your paycheck to pay Iowa state taxes Indiana W-4 form(44-019b) which tells us how much money to withhold from your paycheck to pay Iowa state taxes All required state and federal forms MUST be completed and on file with our main office before we can issue your paychecks.
